Sacrament of the Sick
The Sacrament of the Sick, sometimes known as Extreme Unction, is the Church's special gift of grace and blessing often administered at the point when someone has not long to live. However, this great Sacrament is also one that is used to promote healing for those who are sick in mind and body.

THE SICK IN HOSPITAL
If you know someone whom is going in to or is in hospital from Saint John's Parish, the following information should be given to the Ward staff:

FOR THE ATTENTION OF WARD STAFF
I am a Roman Catholic.
While a patient in hospital, I wish to exercise my Human Right, as enshrined in the European Convention on Human Rights [article 9:1] of practising my Religion.
I request that the Roman Catholic Chaplain be informed of my admission to hospital, and that I receive a visit from the Roman Catholic Chaplain.
I give explicit consent for this information to be made available to the Roman Catholic Chaplain and I would request that this document form part of my personal case notes.
